Rice University requires all degree-seeking students to have health insurance. |
 |
All students, new and returning, must complete their insurance selection at www.studenthealthinsurance.rice.edu for each year they are enrolled at Rice. Students who do not complete an Enrollment or Waiver Form will have their account put on hold. Completed forms should be submitted by the due date on your bill. |

Students electing to enroll in the Rice Student Health Plan may opt to be billed annually or semi-annually. Only newly registered students may waive coverage in the Spring. Enrollment and Waiver elections must be made online and must be submitted by September 4, 2009 (January 2, 2010 for newly registered Spring students). |

Students will have until September 2, 2009 to make any adjustments or changes to their insurance form. |

To ensure accuracy in coverage, always carry your insurance card. |

Enrolling is easy when you follow these simple instructions. |
 |
Begin the process of registering for the Rice Plan at www.studenthealthinsurance.rice.edu. |
 |
Under the dependent coverage section you will see: “Are you planning to enroll a domestic partner?". |
 |
After this selection has been made you will be allowed to continue your individual registration. |
 |
There are three ways to return the signed Affidavit to Aetna:

- In order to maintain compliance with University policy, complete the insurance waiver form at www.studenthealthinsurance.rice.edu.
- Be sure to check your policy to determine which Houston area medical practitioner and facilities are covered under your plan.
- If you are enrolleing in the Aetna Student Health Insurance Plan as an add-on to other health insurance coverage, all claims must be sent to the other insurance company first since the Aetna Student Health plan is secondary.
- It is your responsibility to submit claim forms for each injury or illness. A health provider may offer to submit claims for you, but do not assume that all providers will perform this service.

| The mandatory student health fee is separate from the student health insurance. This fee allows students to receive primary health care, short-term psychological and psychiatric services and health education through the Student Health Services, the Rice Counseling Center and the Wellness Office. Advantage Dental is the optional dental program offered through Aetna. The emphasis of this plan is on preventative care. This plan offers coverage for most routine preventative and diagnostic services, as well as, basic restorative care. Visit www.aetnastudenthealth.com (be sure to enter 890436 as the Policy Number) for more information. Application and payment for this plan should go directly to Aetna it is not administered through Rice. For additional help call Aetna Customer Service: 877-375-7908.

- Coinsurance – The percentage of Covered Medical Expenses payable by Aetna under the Student Health Insurance Plan:
Example: (Rice Plan 80% Preferred Care – 50% Non-Preferred Care).
- Copayment – Those amounts payable by the insured person(s), at the time of service, as set forth in this policy.
- Deductible – A specific amount of Covered Medical Expenses that must be incurred and paid for by the Covered Person before benefits are payable under the Plan. Deductible amounts are the responsibility of the Covered Person. Example: (Rice Plan - $250 in-network $750 out-of-network).
- Covered Person – An eligible person or eligible dependent for which the insurance company receives an enrollment form and proper premium payment has been made, and who is therefore insured under the plan.
- Benefit Period – The policy period from effective date to termination date.
- Medically Necessary – A service or supply that is necessary and appropriate for the diagnosis or treatment of a sickness or injury based on generally accepted current medical practice.
- Preferred Care Provider (or Preferred Provider) – A health care provider that has contracted to furnish services or supplies for a Negotiated Charge.
- Non-Preferred Care Provider (or Non-Preferred Provider) – A health care provider that has not contracted to furnish services or supplies at a Negotiated Charge.
- Out-Of-Pocket Maximum – Individual or Family Out-of-Pocket Limit has been satisfied: Covered Medical Expenses will be payable at 100% for the remainder of the Policy Year. Out-of-Pocket costs include deductibles, copayments and coinsurance. Example: (Rice Plan - $3,000 per insured person).
