  • Aggregate Plan Maximum:  $500,000
  • Annual Deductible:  $250 (Preferred Provider) / $750 (Non-Preferred)
  • Out-of-Pocket Maximum:  $3,000 per insured person
  • Coinsurance:  80% (Preferred) / 50% (Non-Preferred)
  • Prescription Drug Benefit:  A $25 copay for each Brand-Name prescription drug or a $15 copay for each generic. Covered Medical Expenses are payable up to a maximum of $3,000 per policy year.
