If you need to request a copy of your 1095-B, please log into Aetna's Your Member Website (Aetna Navigator) account through Aetna Student Health. If you received a 1095-B with the incorrect social security number, you will need to update your information on your account and the form will be corrected within 5-7 business days. The 1095-B is located in the Letters & Communications tab.
The address on the form for the Employer Sponsored Coverage will show as Burlington, MA. This is correct. If you need to update your address, please contact Student Insurance to update your address.
